1. Consent for Treatment
By filling out the "New Patient Form" or bringing your pet for treatment at our clinic, you authorize Kinslow Veterinary Clinic PLLC and its staff to examine, diagnose, prescribe for, and treat your pet(s). While the veterinary team will provide the best possible care, no guarantee can be made as to the outcome of treatment.
2. Emergency Care
In the event of a life-threatening situation, Kinslow Veterinary Clinic PLLC veterinary staff reserves the right to provide emergency treatment as deemed necessary until the client can be reached. The owner of the pet recieving emergency treatment is finaancially responsible for such care.
3. Payment Policy
Payment is due at the time services are rendered. Kinslow Veterinary Clinic PLLC accepts cash, credit/debit cards, and other approved payment methods. Deposits may be required for certain procedures. Accounts not paid in full may be subject to collections and associated fees.
4. Estimates
Treatment plans and cost estimates may be provided upon request. These are only estimates; actual costs may vary depending on the pet’s medical needs.
5. Abandoned Animals
Under Tennessee Code § 39-14-202, if an animal is left at a veterinary facility for more than ten (10) days after the agreed pick-up date, the animal may be considered abandoned. The clinic has the legal right to rehome the animal in a manner consistent with state law.
6. Deceased Pets
In the event of a pet’s death while under care, the owner will be contacted regarding aftercare options (private or communal cremation, or home burial where legal). If arrangements are not made within thirty (30) days, the clinic reserves the right to proceed with communal cremation.
7. Liability Release
When consenting to treatment of your pet, Kinslow Veterinary Clinic PLLC, its veterinarians, and staff are released from liability for unavoidable circumstances beyond their control, including adverse reactions to treatment, anesthetic risks, or unforeseen complications.
8. Communication
Kinslow Veterinary Clinic PLLC reserves the right to contact clients by phone, text, or email regarding my pet’s medical care, reminders, and billing.
